Grampians Health Horsham - Wimmera Cancer Centre Survivorship Clinic

Supported by a GICS grant, this project was to introduce a sustainable survivorship clinic at the Wimmera Cancer Centre in Horsham.

The projects’ aim was to develop a sustainable survivorship model of care in the Wimmera Cancer Centre (WCC) so that once patients finish active treatment, their needs are identified routinely and they are directed to the supportive care required to keep them well.

The Wimmera Cancer Centre team focused on incorporating a nurse led cancer survivorship clinic into the existing Symptom Management and Education in Cancer Clinic (SMECC) that operates full time in the WCC.
