My Cancer Care Record is a folder of tips and templates, aimed at helping people affected by cancer (patients, carers, families, and support people) to manage information related to their care and treatment – especially multiple treatments provided by multiple people, across different health services, and over long periods of time.

It includes useful questions to ask health professionals and places to record specific details that you may be frequently asked for and find hard to remember.

My Cancer Care Record can also help the healthcare providers who are working with you. It can provide easy access to information that they often require, such as:

  • test results and letters from other hospitals/doctors
  • current medication
  • medical and family history
  • current treatment schedules
  • side effects and symptoms that you have had since your last appointment
  • contact details of other clinicians involved in your care.

It is very important that you understand why you are being asked to take medication and the best way to take it.

Tests help identify your cancer type, find the best treatment, and check how well the treatments are working. Tests can include:

  • blood tests
  • scans such as an MRI, PET or CT
  • biopsy
  • colonoscopy

Test results are usually given to the doctor who ordered the test. In most cases, you can get a copy of your results by asking your doctor or healthcare professional.

TIP: Keeping your test results/reports, helps you:

• remember the dates and results of tests and compare them to old results
• keep track of scans/investigations you have had and where these were done
• show these reports/results to members of your healthcare team

Supports and services are available to help you with those needs beyond your medical or surgical treatments. These services and supports can be provided in the hospital setting and also in the community.

During your care and treatment you may face different medical and treatment-related costs. What you pay will depend on whether you are treated as a public or private patient; the type of hospital/service you attend and the sorts of medication/equipment needs you might have. You may also have to cover the costs of travel and accommodation to attend medical appointments.

It is a good idea to keep a record of your medical and treatment-related costs and travel requirements. This may help you to:

  • claim financial assistance or support
  • claim from your health fund or other insurance providers
  • manage your financial expenses
  • keep records for taxation reasons
  • claim travel and accommodation costs
  • apply for benefits, pensions or reimbursements
  • keep records of when you reach limits such as the Safety Net for Medicare or the Pharmaceutical Benefits Scheme, or your private health fund limits.
