Survivorship care plans at Northeast Health Wangaratta

A project to better support patients transitioning from the end of active treatment into self-management/survivorship care. Providing patients with survivorship care plans to assist and support in addressing their health and wellbeing needs beyond active treatment. Evaluation has been completed with recommendations including scaling up to all tumour streams.
