Survivorship care planning at GV Health

As part of the Victorian Statewide Collaboration to Improve Cancer Survivorship Care project – a strategic partnership between the VICS and the Australian Cancer Survivorship Centre (ACSC) – a survivorship care planning stepped approach pilot project was implemented at Goulburn Valley Health (GV Health).
