Development of a comprehensive Cancer Survivorship Program specific for the diverse community of Northern Health
This project was supported by a NEMICS service improvement grant.
The aim of the project was to develop an overarching cancer survivorship care framework to guide the provision of cancer survivorship care at Northern Health, which the Victorian Cancer Plan 2020-2024 identified as key for improving the wellbeing and support for patients with cancer.
Impact
A framework was developed at Northern Health for the provision of cancer survivorship care which included:
- establishment of policies and processes detailing the provision of cancer survivorship care;
- survivorship care plan templates for each tumour stream;
- efficient tracking methods to identify timepoints relevant to the cancer survivorship program;
- and the development of links with external survivorship services that can be accessible and complementary to internally-provided services at Northern Health.
Outcome
The cancer survivorship framework has been utilised to establish a lymphoma survivorship clinic. The framework will inform future expansion of survivorship services at Northern Health to other tumour streams.
