  • Patient eligibility: From day of diagnosis up to 5 years post treatment completion
  • Referral source: Cancer Care Coordinator, Nursing staff, Community health

Service details

  • Appointment length: 1 hour
  • Service operations: 1-5 patients, 2 days per week
  • Funded service: no
  • Survivorship care providers: Nurses and Allied Health. Interpretation service and aboriginal liaison team available upon request.
