Service eligibility

  • Age: <18 year old
  • Patient eligibility: more 5 years post treatment completion
  • Referral source: Specialist eg. oncologist, haematologist at treating health service

Service details

  • Service operations: >15 patients, 3 days per week
  • Service can cater to urban, rural, or remote populations (statewide service)
  • Funded service: Yes, Grant funding through the Department of Health
  • Survivorship care providers: Nurses, Specialist clinician service eg. Oncologist, haematologist, ​​Allied Health
  • Does your health service record patient survivorship encounters? Yes, through electronic medical record and manual spreadsheets eg. Excel, Microsoft List

Communications with community services: Through electronic medical record, Multidisciplinary meeting, mail-sending letters

Primary focus is late effects of cancer treatment and health, development and wellbeing. By nature of the program we identify psychosocial concerns and signpost/refer to appropriate hospital and/or community supports
