Service eligibility

  • Age: <18 years old
  • Patient eligibility: from the day of diagnosis, up to more than 5 years post treatment completion
  • Referral source: Specialist eg. oncologist, haematologist at treating health service and nursing staff

Service details

  • Appointment length: >1 hour
  • Service operations: 1-5 patients, 1 day per week
  • Funded service: Yes, Philanthropic source
  • Survivorship care providers: Nurses, Specialist clinician service eg. Oncologist, haematologist and nursing staff
  • Does your health service record patient survivorship encounters? Yes, through electronic medical record and manual spreadsheets eg. Excel, Microsoft List

The goal of the Regional Outreach and Shared Care Program (ROSCP) is to provide care as close to home as possible when it is safe and appropriate to do so, this includes cancer surveillance and long term follow up.

If a long term follow-up patient is being seen regionally within the ROSCP – a treatment summary and roadmap will be provided. If required the paediatrician can link families in with local services, supports and resources.
