Service eligibility

• Age: Adult 18+
• as part of current practice all neo/adjuvant patients are given screened and given survivorship pack on finishing treatment. The health service have incorporated this into their current work practice
• Survivorship care providers: Nursing staff

Service details

• Appointment length: 1 hour
• Service operations: 1-5 patients, 5 days per week
• Does your health service record patient survivorship encounters? No, as we don’t have a dedicated survivorship program. We will flag patients that are nearing treatment end for neo/adjuvant treatment. We give survivorship pack which contains flyers and information from the Australian Cancer Survivorship Centre (ACSC) as well as local services. Patients will also have supportive care screening tool redone on completion of treatment to assess any other needs and refer to other agencies if necessary
