Service eligibility

  • Age: Adult 18+
  • Patient eligibility: From the day of diagnosis, up to 5 years post treatment completion
  • Referral source: Specialist eg. oncologist, haematologist
    • ​​GP referral
    • Cancer Care Coordinator
    • Nursing staff
    • Community health
    • Patient able to self-refer

Service details

  • Service operations: 11-15 patients, 3 days/week
  • Service can cater to urban, rural, or remote populations
  • Does your health service record patient survivorship encounters? Yes, through electronic medical record
  • Funded service: No
  • Survivorship care providers: nurses, allied health

Communications with community services via Phone call, Fax, Email

 

Utilising NCCN Distress Thermometer and Problem List, the service provides assistance with addressing:

  • Long term side effects of cancer treatment
