Service eligibility

  • Age: Adult 18+
  • Patient eligibility: from the day of diagnosis up to 1 year post treatment completion
  • Referral source: ​​
    • Specialist eg. oncologist, haematologist
      ​​GP referral
      Cancer Care Coordinator
      Nursing staff
      Community health
      Patient able to self-refer

Service details

  • Appointment length: 1 hour
  • Service operations: >15 patients, 2 days per week
    • waitlist: 1-2 months

  • Does your health service record patient survivorship encounters? Yes, Manual spreadsheets eg. Excel, Microsoft List and patient list through electronic medical record

Using ​NCCN Distress Thermometer and Problem List,​ Quality of life screening tools eg. UW-QOL the service can address:

  • Long term side effects of cancer treatment
  • ​Short term side effect of cancer treatment

  • Physical concerns eg. lymphoedema
