Cancer survivorship care at Kyneton Health

This service can be in a form of multidisciplinary inpatient screening for support needs, inpatient allied health support, weekly outpatient support programs

Service eligibility

  • Age: Adult 18+
  • Patient eligibility: From day of diagnosis up to 5 years post treatment completion
  • Referral source:
    • Specialist eg. oncologist, haematologist
      ​​GP referral
      Nursing staff
      Community health
      Patient able to self-refer

Service details

  • Appointment length: 1 hour
  • Service operations: >15 patients, 3 days per week
    • 2 weeks waitlist

  • Survivorship care providers:Nurses,  Specialist clinician service eg. Oncologist, haematologist,  ​​Allied Health

Utilising ​in-house supportive oncology screening tool (SOST), Patient Health Questionnaires (PHQ-5) and/or Malnutrition Screening Tool (MST) the service can address:
