Service eligibility

  • Age: Adult 18+
  • Patient eligibility: From the day of diagnosis up to 5 years post treatment completion or during treatment maintenance
  • Referral source: Cancer Care Coordinator, Nursing staff, Patient able to self-refer

Service details

  • Appointment length: >1 hour
  • Service operations: 1-5 patients, 1 day per week
  • Does your health service record patient survivorship encounters? Yes, utilising Manual spreadsheets eg. Excel, Microsoft List
