Colorectal Follow up service

Who can access the service?

  • Age Demographic: 18 – 25 years old, Adult 18+
  • When? Post curative treatment
  • Patient eligibility: more than 5 years post treatment completion
  • Who can refer me? Specialist eg. oncologist, haematologist. Discussion about survivorship is briefly covered during surveillance appointments post treatment

Service details:

  • Appointment times: <30 minutes
  • Service operations: >15 patients, 3 day per week

How does the clinic/s keep up with patients? Patient list through electronic medical record

  • Does your health service record patient survivorship encounters? No as this is part of daily activity
  • Does your survivorship the service/s receive dedicated resourcing/funding? No

Communications with community services: Online application or portal, Fax, Email, Through electronic medical record
