  • Age: Adults 18+
  • Patient eligibility: From the day of cancer diagnosis up to >5 years post treatment completion
  • Referral source:
    • Nursing staff
    • Specialist eg. oncologist, haematologist
    • Patient able to self-refer
    • Wimmera cancer centre
    • The Ballarat Austin Radiation Oncology Centre (BAROC)

Service details

  • Appointment length: 1 hour
  • Service operations: 1-5 patients, 5 day a week
  • Funded service: No
  • Survivorship care providers: Nurses, allied health

Communications with community services: Email, Multidisciplinary team meeting

Utilising NCCN Distress Thermometer and Problem List, ​the service can address:
