Service eligibility

  • Age: Adult 18+
  • Patient eligibility: from the day of diagnosis up to 1 year post treatment completion
  • Referral source: Specialist eg. oncologist, haematologist or Cancer Care Coordinator
    • Automatic referral upon diagnosis
    • Patient able to self-refer

Service details

  • Appointment length: 30-45 minutes
  • Service operations: 15 patients, 2 days per week
  • Does your health service record patient survivorship encounters? Yes, utilising manual spreadsheets eg. Excel, Microsoft List, and through electronic medical record
  • Funding source: Hospital budgets eg. VINAH, NWAU, MBS
  • Survivorship care providers:  Nurses,  Specialist clinician service eg. Oncologist, haematologist

Communications with community services: Phone call, Multidisciplinary meeting
