Dedicated outpatient survivorship clinic

Service eligibility

  • Age: Adult 18+
  • Patient eligibility: up to 5 years post treatment completion
  • Referral source: ​​Specialist eg. oncologist, haematologist, Cancer Care Coordinator, Nursing staff, Patient able to self-refer
  • *currently have 4-6 weeks waitlist

Service details

  • Appointment length: >1 hour
  • Service operations: 1-5 patients, 1 day per week
  • Funding source: Hospital budgets eg. VINAH, NWAU, MBS
  • Does your health service record patient survivorship encounters? Yes, electroically and using manual spreadsheets eg. Excel, Microsoft List

Communications with community services: Phone call, Fax, Email

  • Practical concerns eg. transport, housing, accommodation

  • Physical concerns eg. lymphoedema

Provides Our Mob – indigenous resources, Aboriginal Health Unit, Australian indigenous Health info net CCV- resources in other languages, translator service, Peter Mac Cancer survivorship centre resources
