Age: Adult 18+
Patient eligibility: From the day of diagnosis, up to 1 year post treatment completion
Referral source:
- Specialist eg. oncologist, haematologist
- GP referral
- Cancer Care Coordinator
- Nursing staff
- Patient able to self-refer
