Who can access the service?

  • Age Demographic: <18 year old
  • When: Post curative treatment or during treatment maintenance
  • Patient eligibility: more 5 years post treatment completion
  • Who can refer me? Specialist eg. oncologist, haematologist

Service details:

  • Service operations: 11-15 patients, 3 days per week
  • Service can cater to urban, rural, or remote populations

How does the clinic/s keep up with patients? Manual spreadsheets eg. Excel, Microsoft List, Patient list through electronic medical record

  • Does your health service record patient survivorship encounters? Yes, electronic
  • Does your survivorship the service/s receive dedicated resourcing/funding? Yes, Government programs

Communications with community services: Through electronic medical record eg. inbasket, Multidisciplinary meeting, mail-sending letters

Using PAT, other paediatric specific tools, distress thermometer and/or survivorship screening tool, health professionals can address:

  • Long term side effects of cancer treatment
  • ​Short term side effect of cancer treatment
  • Psychological concerns
  • Social concerns
  • Family concerns
  • Physical concerns eg. lymphoedema

  • ​​Tailored cancer survivorship care plan
  • Important contact details
  • Treatment summary (eg. chemotherapy, surgery, radiotherapy)
  • Health service generated resources
