Service eligibility

  • Age: Adults 18+
  • Patient eligibility: Post curative treatment completion or during treatment maintenance, up to 5 years after treatment completion
  • Referral source: Specialist eg. oncologist, haematologist

Service details

  • Appointment length: 1 hour
  • Service operations: 1-5 patients, 1 day a week
  • Funded service: Yes, Hospital budgets eg. VINAH, NWAU, MBS
  • Survivorship care providers:  Nurses
  • Does your health service record patient survivorship encounters? Yes, using manual spreadsheets eg. Excel, Microsoft List, and patient list through electronic medical record

Utilising NCCN Distress Thermometer and Problem List, Tumour specific screening tool, Patient Activation Measure (PAM), Cardiovascular risk calculator and/or osteoporosis risk screening screening tools, ​the service can address:
