Wellness and Exercise Program at Northeast Health Wangaratta

  • Age: Adult 18+
  • Patient eligibility: From the day of diagnosis
  • Referral source: Patient able to self refer to service

  • Service operations: 8 week exercise program hosted by Cancer Council Cancer Wellness and Exercise Program
  • Funded service: Yes, Hospital budgets eg. VINAH, NWAU, MBS
  • Survivorship care providers: allied health

Communications with community services: Phone call, Online application or portal, Email, Multidisciplinary meeting

Utilising NCCN Distress Thermometer and Problem List, the service can assist with:

  • Provide a tailored survivorship care plan
  • Long term side effects of cancer treatment ​
  • Short term side effect of cancer treatment
  • Lifestyle / wellness programs ​​
  • Information for patients
  • Psychological concerns
  • Social concerns
  • Family concerns
  • Physical concerns eg. lymphoedema
