  • Age: Adult 18+
  • Patient eligibility: From the day of diagnosis up to 5 years post treatment completion
  • Referral source:
    • Specialist eg. oncologist, haematologist
    • ​​GP referral
    • Cancer Care Coordinator
    • Nursing staff
    • Community health
    • Patient able to self-refer
