Service eligibility

  • Age: < 18 years old and Adult 18+ (Adolescence and Young Adult service available)
  • Patient eligibility: Post curative treatment, up to 5 years post treatment completion
  • Referral source: Specialist eg. oncologist, haematologist, ​​GP referral, Patient able to self-refer

Service details

  • Appointment length: 30-45 minutes
  • Service operations: 6-10 patients, 1 day per week
    • waitlist: 10 months
  • Funded service: Yes, Hospital budgets eg. VINAH, NWAU, MBS
  • Survivorship care providers: Nurses,  Specialist clinician service eg. Oncologist, haematologist
