  • Age: Adult 18+
  • Patient eligibility: From the day of cancer diagnosis up to 1 year
  • Referral source: ​​
    • Specialist eg. oncologist, haematologist
    • Cancer Care Coordinator
    • Nursing staff
    • Community health
    • Patient able to self-refer

  • Appointment length: 1 hour
  • Service operations: >15 patients, 2 days per week
