Service eligibility

  • Age: Adult 18+
  • Patient eligibility: From the day of diagnosis up to 5 years post treatment completion
  • Referral source: Specialist eg. oncologist, haematologist, ​​GP referral, Cancer Care Coordinator

Service details

  • Appointment length: <30 minutes
  • Service operations: >15 patients, 5 days per week

  • Funded service: no, part of daily activity
  • Survivorship care providers:  Nurses,  Specialist clinician service eg. Oncologist, haematologist,  ​​Allied Health
