Age: Adult 18+
Patient eligibility: From day of diagnosis up to more than 5 years post treatment completion
Referral source:
- Specialist eg. oncologist, haematologist
- GP referral
- Nursing staff
- Community health
- Patient able to self-refer

Appointment length: 1 hour
Service operations: 10-15 patients, weekday
