Service eligibility

  • Age: Adults 18+
  • Patient eligibility: From the day of diagnosis, up to more than 5 years post treatment completion
  • Referral source:
    • Specialist eg. oncologist, haematologist
      Cancer Care Coordinator
      Nursing staff
      Patient able to self-refer

  • Appointment length: 30-45 minutes
  • Service operations: >15 patients, weekday
