Service eligibility

  • Age: < 18 years old
  • Patient eligibility: From the day of diagnosis up to 3 months post treatment completion
  • Referral source:
    • Specialist eg. oncologist, haematologist
      ​​Automatic referral or blanket referral
      Cancer Care Coordinator
      Nursing staff
      Patient able to self-refer

Service details

  • Appointment length: 30 minutes
  • Service operations: >15 patients, weekdays
  • Funded service: Utilising existing resources
  • Survivorship care providers: Nurses, Specialist clinician service eg. Oncologist, haematologist, ​​Allied Health
  • Does your health service record patient survivorship encounters? Yes, using electronic medical record

Communications with community services:  Phone call, Through electronic medical record

Utilising NCCN Distress Thermometer and Problem List, ​Quality of life screening tools eg. UW-QOL the service can address:

  • Long term side effects of cancer treatment
