Type of Service Dedicated outpatient survivorship clinic, Nurse-led service

Tumour Stream Breast, Gynaecology, Haematology, Prostate

Mode of Delivery Face to face appointments, Video call/ telehealth

Age: Adults 18+
Patient eligibility: From the day of diagnosis, up to more than 5 years post treatment completion
Referral source:
- Specialist eg. oncologist, haematologist
- Cancer Care Coordinator
- Nursing staff
- Patient able to self-refer

Appointment length: 1 hour
Service operations: 6-10 patients, 2 days a week
Funded service: Yes, Hospital budgets eg. VINAH, NWAU, MBS
Survivorship care providers: Nurses
Does your health service record patient survivorship encounters? Yes, utilising Paper forms, Manual spreadsheets eg. Excel, Microsoft List, Patient list through electronic medical record

Communications with community services: Email, Multidisciplinary meeting, Mail

Utilising NCCN Distress Thermometer and Problem List and/or K-10 tools, the service can address:
