Who can access the service?

  • Age Demographic: Adult 18+
  • When? From the day of cancer diagnosis
  • Patient eligibility: up to 5 years post treatment completion
  • Who can refer me? Specialist eg. oncologist, haematologist, ​​GP referral

Service details:

  • Appointment times: <30 minutes
  • Service operations: >15 patients, 1 day per week
  • Service available for any priority populations (e.g., CALD communities, Aboriginal and Torres Strait Islander
