Service eligibility

  • Age: Adult 18+
  • Patient eligibility: from the day of diagnosis up to 5 years post treatment completion
  • Referral source: Specialist eg. oncologist, haematologist, ​​GP referral, Nursing staff, Patient able to self-refer

Service details

  • Appointment length: 1 hour
  • Service operations: 1-5 patients, 3 days per week
