Service eligibility

  • Age: Adults 18+
  • Patient eligibility: Post treatment completion up to 5 years
  • Referral source: Specialist eg. oncologist, haematologist, ​​GP referral, Nursing staff

Service details

  • Appointment length: 30-45 minutes
  • Service operations: 1-5 patients, 1 day a week
  • Funded service: Yes, Hospital budgets eg. VINAH, NWAU, MBS
  • Survivorship care providers:  Nurses,  Specialist clinician service eg. Oncologist, haematologist
  • Does your health service record patient survivorship encounters? Yes, using manual spreadsheets eg. Excel, Microsoft List, and patient list through electronic medical record

With the support of NEMICS and VICS Survivorship Program, a project to establish survivorship clinic for allogeneic bone marrow transplant patients is underway.
