The Victorian Cancer Plan 2024–2028 commits to driving progress towards more equitable health outcomes for all Victorians.
As the population of cancer survivors continues to grow, due to advancements in cancer detection and treatment, there is an increasing need for survivorship care that addresses the unique challenges these individuals face. Cancer survivorship is a phase of care that follows primary treatment for cancer. Survivorship can include support for rehabilitation, help to detect and prevent new or recurrent cancers, psychological support, and community-based support (VCP 2024–2028, p.4).
In Victoria, many health services provide survivorship care, but the models of care used vary in practice and approach. The VICS Cancer Survivorship Program 2025–27, in partnership with our member health services, aims to support the scale and spread of established cancer survivorship models across VICS member health services and enhance survivorship care across Victoria.
Implementing survivorship models of care
In March 2025, the program working group engaged VICS member health services to conduct an environmental scan of established cancer survivorship models of care across Victoria. This exercise informed the development of an online directory – the Cancer Follow-up Care Services Catalogue.
This directory will help health services to take the lead in shaping and improving survivorship care models that cater to the unique needs of cancer survivors.
Pilots of survivorship care projects have commenced and are expected to be completed in late 2026.
