WISE - Wellbeing & Integrated Supportive Care Expansion

Grampians​

This project aims to support the spread, scale and re-establishment of supportive care interventions across the Grampians region

​​Supporting oncology patients in a holistic model of patient-centred wellness not only improves patient experience and quality of life, but also supports more successful treatment outcomes. Services including Allied Health, education, social support and complementary care are readily available in cities and major regional centres.

The goal of this program is to facilitate expansion of services for those affected by cancer in rural Western Victoria to participate in a variety of wellness activities close-to-home.​

Activities

  • Local staff training
  • Upgrade AV equipment and staff skills for hybrid / online delivery
  • Stakeholder engagement and communication
  • Identify strategies / tools to improve referral pathways
  • Patient referral pathway data mapping
  • Identify / engage local complementary care providers

Outputs

  • Cancer Council Victoria Education programs and Coping with Cancer running at multiple health services and online
  • Program Reports & Evaluation
  • Database and/or a directory of GICS endorsed services
  • Interactive referral pathway map
  • Wimmera Cancer Centre (WCC) timetable for complementary activities
