Timely access to Prostate Care: Streamlining Triage and Intake for Better Patient Outcomes

In 2025, this project was undertaken to develop a structured triage system for prioritising and managing new prostate cancer referrals, while enhancing the overall intake process for the prostate service. The project was delivered in collaboration between HRICS and the Prostate Cancer Specialist Nurse at Albury Wodonga Health.
A redesign methodology was applied, incorporating quantitative and qualitative approaches. Data collection included stakeholder consultation, process and pathway mapping, and analysis of current workflows to identify key challenges and opportunities for improvement.
Through this work, three key solutions were identified and implemented:
– Referral prioritisation tool to support consistent, clinically informed triage and clearer prioritisation of incoming referrals
– Patient intake form to provide structure and improve completeness and quality of information captured at the first consultation
– Patient referral intake database to improve visibility of referrals, support tracking and enable more proactive management of patients
Early evaluation findings demonstrate positive impacts, including improved service efficiency, more streamlined workflows, and increased consistency in referral and triage decision-making. These changes aim to support timely access to care and enhance the experience for both patients and clinicians.
