Symptom & Urgent Review Clinic (SURC) Optimisation Project
This project aims to review and optimise the existing Model of Care within the Symptom & Urgent Review Clinic
GICS have awarded a Service Improvement Grant to Grampians Health – Ballarat to optimise the Symptom & Urgent Review Clinic (SURC).
SURC was established in 2019 to provide timely, coordinated support for oncology patients experiencing treatment-related side effects or toxicities that do not warrant Emergency Department (ED) care. Therefore, SURC plays a critical role in reducing avoidable hospital presentations, improving clinical outcomes and ensuring continuity of care through a nurse-led, oncology informed model of care.
The current SURC model of care is no longer representative of the service it was designed to deliver. Due to misunderstandings of how SURC services should be utilised, SURC have been receiving referrals to address issues outside the clinics’ scope.
This project aims to review and refine the SURC Clinical Guidelines to ensure they accurately reflect the services’ intended purpose and support equitable workloads of staff. Alongside this, the SURC referral pathways will be updated to align with current and future service demands. This will ensure clarity of scope, improve referral accuracy and support a proactive model of care that prioritises timely, patient centred interventions.
