Cancer Council Cancer Wellness and Exercise Model of Care at Yarrawonga Health

Supported through the HRICS 2025 MK Memorial Grants Program, this project established and implemented a local Cancer Wellness Program to improve access to survivorship support for people affected by cancer in the Yarrawonga community. Through stakeholder consultation, service mapping, evidence review and partnership with Cancer Council Victoria, the project developed a sustainable model of care tailored to local needs. Key outputs included training staff as Cancer Council Victoria facilitators, establishing referral pathways, securing program resources and implementing the first local Cancer Wellness Program. The inaugural program was delivered to 15 cancer survivors and one carer, with the initiative now embedded within Yarrawonga Health’s Community Health Program and continuing to expand with a Cancer Wellness and Exercise Program.
