Nurse-Led Survivorship Clinic at Central Gippsland Health Service
The Nurse-Led Survivorship Clinic at Central Gippsland Health Service supports people transitioning from cancer treatment to long-term recovery through coordinated, personalised care.
Survivorship care can be inconsistent and fragmented, particularly in regional areas where access to coordinated services is limited. This project addresses that gap by introducing a structured, nurse-led model of care that provides personalised, holistic support for patients after treatment.
The clinic plans to deliver coordinated follow-up care, including individualised survivorship care plans, health education, ongoing monitoring, and referrals to allied health and psychological services. Patients are supported through a collaborative approach involving oncology teams, general practitioners, and specialist services to ensure continuity of care.
With the support from VICS Survivorship Program, the clinic aims to improve patient outcomes and wellbeing by enhancing care coordination, increasing patient engagement, and ensuring consistent follow-up after treatment. Long term, the program seeks to embed survivorship care as a standard part of cancer services, creating a sustainable, patient-centred model for regional healthcare.
