Navigating oncology survivorship services in the Macedon Ranges

Three health services in the Loddon Mallee Region have been successful in their applications for VICS-funded survivorship projects.

Navigating oncology survivorship services in the Macedon Ranges – referral pathways between acute services and the community to facilitate long-term survivorship care, Central Highlands Rural Health – Kyneton’s (CHRH) project is to create a database of survivorship services in the Macedon Ranges and strengthen pathways from acute hospital services to outpatient and community-based supports.

This will serve as a tool for staff to utilise, to ensure survivors are able to access quality care within the Macedon Ranges and reduce the need for additional travel. The project also aims to formalise the Allied Health Navigator Role which is currently an informal role shared by the Allied Health team at CHRH.
