MCH Nurse Practitioner-led Survivorship Clinic
A nurse practitioner-led survivorship clinic: supporting Monash Children’s Hospital paediatric oncology patients at the end of treatment.
The Monash Children’s Hospital (MCH) Cancer Centre provides comprehensive cancer care for children and adolescents, however there is significant opportunity to improve holistic care for patients and families in the period between finishing treatment and transfer to the statewide Long Term Follow-up Program (LTFP) or adult services.
With the support of the Paediatric Integrated Cancer Service (PICS), MCH will develop and pilot a nurse-led survivorship clinic at the Children’s Cancer Centre incorporating physical examination, psychosocial support and allied health involvement. The clinic aims to increase access to personalised treatment summaries and roadmaps, facilitate early detection and management of cancer related physical, psychosocial, and developmental issues, and provide timely referrals to appropriate services for input and assessment.
The clinic aims to increase access to and utility of resources and services during the surveillance period, to ultimately improve families’ experiences, knowledge, and ongoing self-management.
