Let's Yarn about it While We Can - My Wishes: A Guide for My Mob
Supported through the HRICS 2026 MK Memorial Grants Program, this project established a comprehensive evidence base to better understand the care needs and service pathways of people with advanced cancer living in residential aged care facilities across the Goulburn Valley. Through retrospective data analysis, stakeholder consultation and client journey mapping, the project identified opportunities to strengthen anticipatory care planning, education, referral pathways and multidisciplinary collaboration, while demonstrating the value of the existing specialist palliative care nurse-led aged care model. A key output was the development of five evidence-informed recommendations focusing on anticipatory care planning, education, referral pathways and multidisciplinary collaboration to support future service improvement.
