Home-Based Cancer Care Review at Barwon Health
BSWRICS is funding a 12-month project position to support Barwon Health in undertaking a comprehensive review its Home-Based Cancer Care Program.
Established in 2016, the Home Based Cancer Care Program has grown significantly in response to advances in cancer treatment delivery and increasing consumer preference for care closer to home. Home-based models of care have demonstrated benefits including improved patient experience, reduced demand on hospital services, and clinical outcomes comparable to traditional hospital-based care.
As home-based cancer care continues to evolve, there is an opportunity to review current service delivery and identify opportunities to strengthen consistency, governance, integration and sustainability across the program.
Project Aim
The review will evaluate the effectiveness, quality, safety and value of current home-based cancer care services. Findings will inform evidence-based recommendations to enhance future service delivery and support the ongoing development and potential expansion of the home-based care.
Project Objectives
The project will:
- Engage key stakeholders and establish Project Advisory Group (PAG)
- Develop a detailed project plan, program logic and methodology
- Conduct a comprehensive review and evaluation of the current HBCC program.
- Current state assessment report
- Options and recommendations outlining future state, service redesign models and implementation roadmap
- Support prioritisation and implementation planning of endorsed recommendations to transition to an optimised and sustainable model of care.
